Student Accident Insurance
For students who are not covered by a family accident insurance plan, parents should consider purchasing one of the student accident insurance plans available through the district.
How to File a Claim
- If you have purchased Student Accident Insurance and need to file a claim, use the Student Accident Insurance Claim Form (PDF) (Seguro de enfermedades y accidentes para estudiantes (PDF).

About the Student Accident Program
The Student Accident Program:
- Is available to all enrolled APS students at no cost
- Covers enrolled students for all school-time activities, including interscholastic sports
