APS COVID Warehouse Ordering

Please download a new form for every order you place. We make changes to the form frequently, and we want you to use the most up-to-date form.

No returns on PPE/COVID related items.

Materials Management has set up a separate warehouse for COVID related items covered under the CARES Act. These items are available to all APS schools, and select departments. Please print out the COVID order form, and fill in the school/site, location number, contact name and a good phone number to reach them at. Please email these forms to stock.control@aps.edu.

You may still acquire Hand Soap, Bleach, and Paper Towels by purchasing these items from Materials Management Supply Warehouse.
