After School Snack Program Checklist

Required Documents for the After School Snack Program
Document	Requirement
Training Module	Prior to receiving snacks, all program staff members who will be responsible for both distributing snacks, and the completion of all documentation, must complete the appropriate training module.
Application	Prior to receiving snacks, one application per program must be submitted.
Questionnaire Review	All program staff members who will be distributing snacks must complete and submit the Questionnaire Review. This is an annual requirement. On School Sites need to train 2 staff members and submit a Questionnaire Review. Off School Sites need to train 4 staff members and submit a Questionnaire Review.
Tally Sheet	Required for all programs who serve more than 25 students.

On-Site After School Snack Training Module (PDF, 10.2 MB): For organizations that are consuming snacks at the school site

Application

Program Application (PDF, 3 MB)

Program Site Leader must complete and submit an After School Snack Application.

Approval is given within 2 to 3 working days if the After School Program Questionnaire Review for each Program Official has been verified.

Program Official must complete and submit an After School Snack Program Questionnaire Review.
