Request for Remote Work Assignment

Information about requests for Americans with Disabilities Act (ADA) Accommodation associated with the COVID-19 pandemic.

About the Process

The following information is intended to assist you in your application for accommodation and/or to inform you of the process to formally apply for a remote assignment.

Who Can Apply for Accommodations

If you are an individual with any of the conditions identified by the Centers for Disease Control and Prevention as being at “high risk” of experiencing severe illness should you contract the COVID-19 virus, you may apply for an ADA accommodation.

Types of Accommodations

ADA High-Risk (COVID) Request for Accommodation

If you are interested in ADA accommodation, submit the ADA Employee Request for Accommodation form. ADA accommodations associated with COVID-19 include, but are not limited to:

  • enhanced PPE (gowns, face shields, gloves, barriers around work area)
  • temporary elimination of marginal job functions/opportunity to perform marginal functions remotely where possible
  • virtual meeting attendance
  • increased space between teaching area and student seating
  • move to a larger learning environment such as a gym or auditorium (when available/feasible)
  • installation of additional sanitizer stations, and
  • assignment of janitorial staff to clean and sanitize work area more frequently.

In rare circumstances, reassignment to remote work or a leave of absence may be approved as an ADA accommodation.

ADA Employee Request for Accommodation

If you are interested in ADA accommodation, submit the ADA Employee Request for Accommodation form. You must include the following information  in the form:

  • Full Name
  • Job Title
  • Contact Information
  • School or other APS location where you work
  • Name and email address of your immediate supervisor; and
  • Attach a medical certification from your treating physician that clearly identifies the high risk condition.
