Document Remediation Request Form
Use this form to submit your document remediation request.
Accepted formats:
- MSWord, Powerpoint, Excel, or Google.
- Simple forms. Complex forms require a longer turnaround time.
- Other types of documents will require review from the Document Technician before acceptance.
Formats not accepted:
- Scanned documents or PDF files of scanned documents.
- Files created in Adobe InDesign will be considered on a case-by-case basis.
- Complex documents, including complex forms, or documents with many complex tables.
