Form Instructions

  • Must be completed by APS Employees only.
  • This order form is for ordering new devices or device upgrades only.
  • All fields are required.
  • IMPORTANT! After you submit your order, you will receive a confirmation email with your order total at the email address provided in the form. 

How to Complete Your Order

  1. Submit the online order form.
  2. Fill out your Requisition Request.
  3. Email your Requisition Number to smartphones@aps.edu.

To order accessories email your request to smartphones@aps.edu.
