Form Instructions

  • Please only submit one request per area of support from the Area of Support list below.
  • You will receive a confirmation email of your request at the email address you provide.
  • All fields marked with an asterisk (*) are required.
  • Attention Principals - If you are submitting a ticket requesting Synergy access for a user, we need your information in the "Employee Information" section, and the user's information in the "Describe Your Issue" section. Without principal information, the ticket will be sent back requesting principal approval. Thank you!
