School-required immunizations at no cost for children ages 4-18.

Bring your child’s immunization record if available.

• No cost (bring insurance information if covered)
• Onsite registration
• Open for any school aged child in the community ages 4-18
• Parent/Guardian must be present

Free Vision Exams provided by The Lions Club and free Dental Exams provided by Blue Cross Blue Shield.

Required immunizations for K-12:

• 4 or 5 doses of Diphtheria-Tetanus-Pertussis (DTaP)
• 3 doses of Hepatitis B (Hep B)
• 3 or 4 doses of Polio (IPV)
• 2 doses of Measles-Mumps-Rubella (MMR)
• 2 doses of Varicella
• 2 doses of Hepatitis A (Hep A)

Additional required immunizations for 7th-12th grade entry:

• 1 dose of Tetanus-Diphtheria-Pertussis (Tdap)
• 1 dose of Meningococcal (Men ACWY) (required 7th-10th grade; 2 doses for 11th grade, 2nd dose age 16 yrs+)
• (Not required; highly recommended)
• 2-3 doses of Human Papillomavirus (HPV)
