APS School Based Health Centers

School Based Health Centers (SBHC) are health clinics located on or near school property that will provide health care for any APS student.

The SBHC Program

  • APS has twelve School-Based Health Centers (SBHC).
    • East San Jose Elementary
    • Emerson Elementary
    • Grant Middle School
    • Van Buren Middle School
    • Washington Middle School
    • Wilson Middle School
    • Albuquerque High School
    • Atrisco Heritage Academy
    • Highland High School
    • Manzano High School
    • Rio Grande High School
  • These student-based programs promote comprehensive health services to students at school so they can avoid health-related absences and receive support to succeed in school.
  • They deliver a variety of quality, integrated, youth-friendly, culturally responsive services like behavioral health, primary health, dental, health education, prevention and wellness care.

Find a SBHC Site

Locate a SBHC site on the New Mexico Alliance for School-Based Health website. The following information is listed by town and then by school:

  • Clinic phone number.
    • Contact the SBHC nearest you for direct information on how to access services.
  • Services offered
  • Days and hours of operation
  • Grades served (elementary SGHC will serve elementary students, middle school SBHC will serve middle school students, and high school SBHC will serve high school students)

A printable PDF version of the site directory is also available for download.
