DACA Documentation Requests
This form is for persons who need documents from Albuquerque Public Schools for verifying to the Department of Homeland Security - U.S. Citizenship and Immigration Services that they came to the U.S. before their sixteenth birthday.
What APS can provide:
- Dates of attendance for students who attended an APS school in any grade kindergarten through 12th, and who were born after 1980.
- Transcripts of grades can be provided for those who attended an APS middle school or high school, and who were born after 1980.
What APS cannot provide:
- If students attended another public school, charter school or private school, they will need to request documents directly from those schools.
- If students received their General Equivalency Diploma (GED), they will need to request these records from the state in which the diploma was issued.
Form Instructions
- Please only submit one request per person. You will receive a confirmation email containing the information submitted on this form at the email address you provide.
- This form applies only to people who have attended APS and were born after 1980.
- All fields are required.
