Required Weekly Surveillance Testing for APS Employees

APS employees are required to participate in weekly COVID-19 surveillance testing if they aren't fully vaccinated or don't complete the testing exemption survey.

Public health orders require all public, private, and charter school employees either be fully vaccinated for COVID-19, including a booster if eligible OR undergo weekly coronavirus testing.

Who is Exempt from Testing

  • Employees who are fully vaccinated, including a booster if eligible,  AND have completed the COVID-19 Testing Exemption Survey To complete the survey, you need to be logged into Google and use your APS Gmail account. Employees may be asked for proof of vaccination.
  • Employees who have tested positive for COVID-19. These employees are exempt from surveillance testing for 90 days from when they last tested positive. They should notify their supervisor of their positive test date for this exemption. 
  • Unvaccinated and partially vaccinated employees exposed to COVID-19 and participating in Test to Stay (only during the week they test through the program). 

Who Isn't Exempt from Testing 

  • Employees who aren’t fully vaccinated
  • Employees who choose not to disclose whether they are vaccinated
  • Employees who fail to complete the survey

Employees required to test weekly shall do so on their own time (i.e., outside of the duty day or while on leave during the duty day).

You must send your COVID-19 test result to your supervisor immediately after receiving test results but no later than the Thursday of each testing week.

Directions for Weekly Surveillance Testing

Select one of the three COVID-19 testing options:

  1. Over-the-counter tests (may be taken outside of work) as long as an Over-the-Counter Rapid Antigen Testing Assurance Form is submitted to supervisor verifying the date of test and results.
  2. Rapid tests provided at Department of Health-sponsored testing sites.
  3. Curbside testing at Berna Facio Professional Development Complex conducted by Premier Medical Group:
  • 7:15 a.m.-3:30 p.m. Monday-Friday through June 2. Closed Memorial Day (Monday, May 30). Testing at Berna Facio ends June 2. 
  • Take your test during the assigned testing window; the sooner, the better.
  • Send your test result to your supervisor as soon as possible but no later than the Thursday of each testing week.
