APS Staff and Faculty Daily COVID-19 Questionnaire

Each day before heading to work, please review the following questions regarding your health.

If you answer yes to any of the questions

Stay home and contact your supervisor. It is also highly recommended that you consult a medical provider or call the COVID Hotline at 855-600-3453.

Questions for APS Employees Before Reporting to Work

  1. Do you currently have a temperature of over 100.4 degrees?
  2. In the past 24 hours, have you experienced:
    • Fever
    • Fatigue, muscle or body aches
    • New frequent, dry cough
    • Sore throat, runny nose, congestion
    • Diarrhea, vomiting
    • New onset of headaches
    • Shortness of breath, difficulty breathing
    • Loss of smell or taste
  3. Have you recently been in close contact with anyone who has exhibited any of these symptoms?
  4. Have you recently been in contact with anyone who has tested positive for COVID-19?

If you become ill while at work

  1. Notify your supervisor and leave the worksite immediately.
  2. Do not socialize or enter any other area of the worksite before leaving.
  3. Consult your medical provider or call the COVID Hotline at 855-600-3453.
