Student Accident and Health Insurance
For students who are not covered by a family health or accident insurance plan, parents should consider purchasing one of the student accident, health and athletic insurance plans available through the district.
How to File a Claim
- If you have purchased Student Accident & Health Insurance and need to file a claim, use the Student Accident Insurance Claim Form (PDF).
- You can also contact Myers-Stevens & Toohey & Co., Inc., at (800) 827-4695.
About the Student Accident and Health Insurance Program
The Student Accident and Health Insurance Program:
- Is available to parents of all APS students,
- Provides low-cost accident insurance for students,
- Has options for full year, 24-hour coverage, for school time only, for football coverage and extended dental coverage.
