APS Vaccine Clinic Information

Flu, COVID 19 (vaccine and booster), Pneumonia, Tdap, and Shingles shots are available during the APS Employee Wellness Fair.

About the Vaccine Clinic

The vaccination clinic is Thursday, October 6 from 7:30 a.m. to 3:30 p.m. at the Berna Facio Professional Development Complex, located at 3315 Louisiana Blvd. NE, Albuquerque, NM 87110. Rooms 111 & 112

There will be 2 lines - One for those requesting multiple vaccines and another line for people requesting just one vaccine (of any type). This will expedite the vaccination process.

Appointments

You do not have to make an appointment. You may, however, be waiting in line. This year we will have two separate lines: one for those requestion multiple vaccines and another line for anyone needing just one vaccine (of any type being offered at the fair). Please note that if you have APS benefits you can get your flu and COVID vaccines for free at any major pharmacy or your local primary care physician's office.

Consent Forms

It's always nice to get expedited to the front of the line and that's what will happen if you come with your consent form filled out!  Please download the consent form to your computer, print, fill out, and bring this vaccination consent form with you to this event.

Bring your prescription benefits card or cash.

For employees covered under APS Medical Benefits:

Your shots are completely covered. Bring your Express Scripts card. COVID, Flu, Tdap, Shingles, and the Pneumococcal Vaccines will be offered at no charge for APS members. You may also bring covered spouses, domestic partners, and children.

For employees with insurance outside of APS Benefits:

Bring your prescription benefits or healthcare insurance card. Flu vaccine will be offered at no charge. Pneumococcal and Tdap vaccines will not be offered on-site; however, after the clinic, the Smith’s Pharmacist may be able to help you determine if your benefits cover these vaccines.

If there is an APS employee, or family member, who does not have insurance and would like a vaccine, Smith's offers a clinic price of $30 for a standard flu shot. If you want the high-dose or FluBlok vaccine, that is a cash price of $75. These prices are only for vaccines administered by Smith's at the APS clinic only. Smith's can only take exact cash at the time of the vaccination. They will not be able to process Credit Cards.

Bring the printout of your vaccine status and allow our pharmacist to help you determine if you are up to date on your vaccines!

You can now check your own vaccine status online through the New Mexico Department of Health. The immunization record can be sent to you via text or email.
