Don't Forget to Schedule a Health Screening
Deadline is Sept. 30; appointments are available at schools and on Saturdays at City Centre.
APS employees and their spouses/partners who have health insurance or who would like to enroll in health insurance through the district have the opportunity to keep premiums down for the sixth straight year by participating in the 2016 Employee Wellness Incentive Program.
The program includes biometric health screenings and the completion of a tobacco affidavit. The program also provides many health tools nad resources to help you reach your personal health goals.
Participation is voluntary; however, in order to receive the savings for the new plan year effective January 1, 2016, employees, spouses/partners must complete a biometric health screening and sign a tobacco affidavit by Sept. 30, 2015.
Schedule your screening to avoid long lines as the deadline approaches.
Screenings are from 7 a.m. to noon at school sites and from 6-11 a.m. on Saturdays at City Centre throughout September.
They will be held at the Lincoln Complex from 6-11 a.m. through Friday.
Biometric Screening Information
- All biometric screenings will be administered by licensed health professionals of Health Advocate.
- Screenings include a full lipid panel (total cholesterol, HDL, cholesterol ratio, LDL, triglycerides & glucose), blood pressure, and body composition.
- You will need to fast for 8 hours prior to your screening (only water is permitted).
- Screenings are done via finger stick with results received in minutes and the information will be held confidential.
