My Chronic Diseases Program (MyCD)

The MyCD Program is a free program designed for middle aged, senior, and disabled adults with chronic health conditions such as:

  • Diabetes

  • Arthritis

  • High blood pressure

  • Asthma

  • Heart disease

  • Chronic pain

 

  • Workshops are fun and interactive. Participants share their successes and build a common source of support. The MyCD Program helps build confidence for managing health, staying active and enjoying life. It does not conflict with existing treatment or disease specific education. Participants meet once a week for       2 ½-hours, for six weeks.

  • Workshops are available in English or Spanish.

  • Caregivers and family members are also encouraged to attend.

  • Learn from trained workshop leaders who also have chronic health conditions.

  • Workshops are offered at NO COST.

  • Get a great resource book and relaxation CD upon completion of the MyCD Program.
