2019 Wellness Incentive Program

To keep insurance costs down, employees need to see a primary care provider/practitioner* by Sept. 30.

APS employees and spouses/domestic partners who have health insurance through the district can keep costs down by completing a medical physical sometime between October 1, 2017 and September 30, 2018.

Open enrollment occurs between July and September, where you can upload your documents, in order to get the insurance fee reductions. You must keep your physical documentation in your possession until the open window to upload become available (July-September). A suggestion is to scan and email it to yourself so that it's not lost.

New this year: You are responsible for uploading this form to the Wellness at Work Platform (not your medical provider).

  • Register on the Wellness at Work Platform (open from July 1 - September 30)
  • Upload your completed Physician Engagement Form on the Wellness at Work Platform
  • Complete the online Personal Health Assessment on the Wellness at Work Platform
  • Complete your Tobacco Affidavit on Wellness at Work Platform **Individuals who attest to using tobacco must also complete a 12-week online tobacco cessation course on Wellness at Work to be completed in full by December 30.
