Have You Seen Your Doctor Yet?

To keep insurance costs down, employees need to see a primary care provider/practitioner* by Sept. 30.

*We've added obstetrics and gynecology to the list of qualifying exams if they are identified as a primary care provider/practitioner. 

APS Employee Wellness is sponsoring a wellness incentive program to help keep down insurance costs. Employees and their spouses/domestic partners who have health insurance through APS are encouraged to complete three steps to keep costs from going up $25 a month ($300 a year) for individuals or $50 a month ($600 a year) for employees and their spouses/domestic partners in 2018.

Here's How to Keep Down Your Insurance Costs

1. Complete a preventative health exam sometime between Oct. 1, 2016, and Sept. 30, 2017. There is a ZERO co-pay for annual preventative doctor visits!

Qualifying exams include:

Send questions to 
  • Primary Care Providers from Physicians, Nurse Practitioners or Physician’s Assistants. These are medical professionals who agree to manage and coordinate the care provided to a member in a managed care program and agrees to coordinate health care needs with other care providers.
  • Endocrinologists
  • Cardiologists
  • Oncologists
  • Obstetrics, OB/GYN, Gynecologists (added) – if identified as your primary care provider

Annual exams that do not qualify are:

  • Dermatology
  • Dental
  • Vision
  • Age appropriate screenings such as mammograms and colonoscopies
  • Occupational health or workman’s compensation
  • Exams from other countries

 2. Complete the employee/patient portion of the Physical Engagement Form and present it to your primary care provider to complete. You or your provider's office can fax or email the completed document to the contact information provided on the form. Get the Physician Engagement Form.

APS is using the current third-party administrator, HealthAdvocate to assure confidentially and privacy. APS Employee Benefits will only receive confirmation that the exam was completed.

3. Sometime between July 1, 2017 - September 30, 2017, complete the personal health profile and tobacco affidavit via your wellness portal on the HealthAdvocate website. If you are a tobacco user, you will be encouraged to complete an online tobacco cessation program in order to qualify for the wellness incentive.

The APS Employee Benefits and Wellness Program was developed to educate and assist employees in improving current health risks. While regular visits such as dental, vision, exams for CDL or occupational health/worker’s compensation, and exams from other countries are sometimes instrumental in detecting disease, the district feels that the most effective and time sensitive way to identify chronic risks is through primary physician and specific types of specialist visits.

APS understands the value of regular dental and vision exams, as well as dermatologic screenings. However, primary care and specialist doctors are uniquely qualified to identify conditions through lab work, testing, and diagnostics as well as providing one on one discussions to help understand and manage risks.

While not included in the 2018 Wellness Incentive Program, APS will continue to provide a robust benefits program to provide quality health care coverage for dental, vision, prescription drugs, life insurance, long-term disability, pre-tax insurance premium plan, flexible spending accounts, long-term insurance, and deferred compensation.
