Insured Employees Need to Verify Dependent Eligibility

APS has hired an independent auditor to make sure spouses, domestic partners and children insured through the district meet coverage requirements.

APS employees who have health insurance through the district have been asked to provide documentation verifying the eligibility of dependents enrolled on their plan.

APS is conducting the dependent eligibility verification in an effort to help keep down healthcare costs for its employees. The audit will ensure APS health benefit plans are compliant, competitive and cost effective.

Employees with enrolled dependents should have received a letter from the auditing firm HMS Employer Solutions with information on how to verify eligibility. Those who haven't received a letter should contact Employee Benefits at 889-4859 or employee.benefits@aps.edu.

The documentation needs to be sent to HMS Employer Solutions (not APS Employee Benefits) by Feb. 23.

HMS is Employer Solutions is an independent firm that specializes in verifying health plan eligibility. Personal information will be protected. Only document copies should be sent, and social security numbers, income or account information should be blacked out.

Information should NOT be sent to APS Employee Benefits, nor can APS provide employees with the information to send to the auditing firm. If you have questions, call HMS Employer Solutions at 855-292-1300.

Eligible dependents include:

  • GO HERE for information on obtaining a copy of a New Mexico birth certificate.
  • GO HERE for information on obtaining a copy of a birth certificate outside of New Mexico.
  • Legal spouses
  • Qualifying domestic partners
  • Children up to age 26
  • Children age 26 or over who are incapable of self-support due to a mental or physical disability.

Proof of eligibility includes:

For Spouse:

A copy of the marriage certificate and one of the following:

  • A copy of the front page of the most recently filed federal 1040 tax return OR
  • A document dated within the last 60 days showing both the employee and spouse's names. This may be a recurring household bill or an account statement. This document also must include the date and mailing address.

For Domestic Partners:

  • A copy of the affidavit of domestic partnership AND
  • A document dated within the last 60 days showing both the employee and domestic partner's names. This may be a recurring household bill or an account statement. This document also must include the date and mailing address.

For Children:

  • A copy of the child’s birth certificate or adoption certificate naming the employee and/or spouse as the child’s parent(s). The document must list the first and last names of the child and parent(s) OR
  • A copy of the court order naming the employee and/or the employee’s spouse as the child’s legal guardian(s).
  • For a step child or domestic partner’s child, employees must also provide documentation verifying the current relationship to their spouse/domestic partner.
