Medical Insurance Tax Forms Sent by March 2

The IRS doesn't require that you submit the 1095-C form with your return, nor will you be required to amend your tax return once you receive the form.

The IRS requires that APS distribute 1095-C forms (Employer-Provided Health Insurance Offer and Coverage form) to employees who are enrolled in an APS medical insurance plan.

The 1095-C form includes information about medical plan enrollment for you and any enrolled dependents. The IRS deadline for APS to distribute the 2017 form is March 2.

We realize that some employees will file their 2017 federal income tax return before they receive their 1095-C form. However, the IRS has indicated that you do not need to submit the 1095-C form with your return and that you will not be required to amend your tax return once you receive your 1095-C Form. The Albuquerque Public Schools medical plan is considered ‘qualifying health care coverage’ (also referred to as Minimum Essential Coverage) under the Affordable Care Act.
