Applying for Benefits

Did you know you can apply for some benefits any time during the year?

Many employees assume they have to wait until Switch/Open Enrollment to apply for any of the APS benefit programs. However, there are several benefit plans that eligible employees can apply for at any time during the year.

Additional Life Insurance and Spouse Life Insurance

  • Complete an application for coverage in the APS Employee Benefits Department or online, and then complete the online Medical History Statement. Get the forms.
  • Adding or increasing the amount of life insurance coverage is subject to Evidence of Insurability, which requires the employee or spouse to complete a Medical History Statement for review by The Standard, the company that handles the life insurance plan. Coverage is subject to underwriting by The Standard and is not guaranteed.

Long-Term Disability (LTD) Insurance

  • Complete an application for coverage in the APS Employee Benefits Department or online, and then complete the online Medical History Statement. Get the forms.
  • The addition of LTD coverage is subject to Evidence of Insurability, which requires the employee to complete a Medical History Statement for review by The Standard, the company that handles the LTD plan. Coverage is subject to underwriting by The Standard and is not guaranteed.

Long-Term Care (LTC) Insurance

  • To apply or find out more about this insurance, call (855) 558-1727 or visit the website.
  • LTC insurance is subject to underwriting by LifeSecure, the company that handles the LTC plan. Coverage is subject to Evidence of Insurability which requires the employee or spouse to complete a Medical History Statement for review by LifeSecure; coverage is not guaranteed.

LifeSecure announced this month that they are developing a new LTC insurance product, which they expect to be available in early to mid-2018. Unfortunately, this will result in a temporary disruption to your ability to apply for LTC. If you are interested in considering this insurance, you will need to contact them to request information or apply for coverage prior to November 30, 2017. Otherwise, you will need to wait until the new product is available next year to apply for coverage.

403(b) or 457(b) voluntary retirement savings plans

  • Employees may enroll in the voluntary retirement savings plans at any time during the year. Information about how to enroll is available on the Employee Benefits Department webpage or call (505) 889-4821.
  • All employees, whether full-time, part-time, on an hourly status, or a substitute teacher or substitute educational assistant, are eligible to participate in either of the voluntary retirement savings plans.

If you are interested in any of these plans, you may apply or enroll now, and do not need to wait until Switch/Open Enrollment which is scheduled for October 6-13.
