After School Snack Program

PRIOR TO RECEIVING SNACKS

Every Program must submit an After School Snack Application.

Program officials, who will be responsible for both the distribution of snacks and the completion of all documentation, are to complete the appropriate After School Snack Site Training Module listed below.  All Program Officials must also complete an After School Snack Program Review listed below.  This is an annual requirement.

Program Application to:

  1. Program Site Leader must complete and submit an After School Snack Application.
  2. Approval within 2 to 3 working days if After School Program Questionnaire Review for each Program Official has been verified.
  3. E-Mail, Fax, or Mail completed After School Snack.

  1. Program Official must complete and submit an After School Snack Program Questionnaire review.

After School Snack Program Tally Sheet

Tally Sheet

  1. Required for all programs who serve more than 25 students.
  2. Print Tally Sheet.
  3. Use as instructed on Tally Sheet 

AFTER SCHOOL SNACK PROGRAM CHECKLIST

  1. All After School Snack Programs must complete and submit an After School Snack Program Application.
  2. One Application per program
  3. All program staff members who will be distributing snacks must complete an On Line Training Module.
  4. All program staff members who will be distributing snacks must complete and submit an After School Snack Program Questionnaire Review.
  5. On School Sites will need 2 staff members to train and submit a Questionnaire Review.
  6. Off School Sites will need 4 staff members to train and submit a Questionnaire Review. 

NOTE:  SUBMITTING THE FOLLOWING BY E-MAIL:

  1. After School Snack Program Application: must include  Electronic Signature.

2.  After School Snack Program Questionnaire Review.

E-mail button will work if official has a local e-mail client on their computer.  If using a school lab or library PC and an e-mail system like hotmail, Gmail, APS. Webmail, etc., you will have to save the document to your desktop and attach it to an e-mail.
