1. How do I report a work-related injury? Notify your supervisor and call Company Nurse at 1-877-740-5017 (24 hours a day, 7 days a week). The triage nurse will gather information over the phone and assist in accessing appropriate medical treatment, if necessary. Also submit completed New Mexico Worker's Compensation Administration Notice of Accident form to your supervisor within fifteen (15) days of the accident or injury. 2. Where do I receive medical treatment for a work-related injury? Employees requiring medical treatment must call Company Nurse at 1-877-740-5017 (24 hours a day, 7 days a week). The triage nurse will gather information over the phone and assist in accessing appropriate medical treatment, if necessary. For life-threatening emergencies, call 911 or seek treatment at the nearest hospital emergency facility! 3. How do I report loss or damage to APS property? Submit completed APS Property Loss Report. 4. How do I report a third-party liability claim? Submit completed Tort Claim Notice - Liability. 5. What paperwork must I complete to transport students in a private automobile? You must have approval of the school principal and must complete the Notification of Responsibilities and Liabilities in Transporting Students form - English - Spanish. You must also have a valid driver's license, proof of insurance and volunteers with unsupervised access to students must successfully complete an APS background investigation through Human Resources. 6. When do I need to purchase Activity Trip Insurance? Student activity trip insurance is required student travel sponsored by schools that are non-educational in nature used to reward academic, athletic or behavioral accomplishments. Click here for the Activity Trip Insurance form. 7. How can I purchase Accident & Health Insurance for my child? For students who are not covered by a family health or accident insurance plan, parents can purchase accident and health insurance coverage. This program provides options for low-cost accident insurance. Click here for the Student Accident & Health Insurance form English - Spanish. 8. Am I eligible for employee Hepatitis B vaccination? Certain positions have been identified as eligible for Hepatitis B vaccinations, based on the likelihood of occupational exposure to blood or body fluids. See the APS Bloodborne Pathogens Plan Exposure Determination Summary for more information. If your job category is not among those eligible for Hepatitis B vaccination, you and your active dependents may receive Hepatitis B and other commonly requested vaccinations with no copayment if enrolled in the Express Scripts prescription coverage plan. 9. What are the requirements for reporting fire and emergency drills? Drills must be reported to Risk Management via the APS intranet, click here. |
