ALL MEDICATION MUST BE BROUGHT TO THE HEALTH ROOM BY THE PARENT OR GUARDIAN.

Authorization to Administer Prescription Medication form- Students who take prescription medication at school must have a physician's order on file in the health room for the medication(s).  If the medication is to be carried by the student, self administered by the student, and or needing supervised administration by the health assistant or administered by the nurse the doctor and parent need to indicate this on the form. APS medication forms can be picked up in the school health office or you can download the form from our website.  The order must include the student's name, name of the medication, dose to be taken, the time of administration, and physician signature.  The medication must be in a prescription labeled container that matches the physician's written orderA separate form must be filled out for EACH medication. If there is a dosage change in a medication, a new order is required. At no time will a student be allowed to carry a medication classified as a controlled substances (example: Ritalin, Dexedrine, Tylenol with Codeine, etc.). 

Parent Authorization For Over-The-Counter Medication form- Students who take over the counter medication while at school must have this form on file in the health room.  This form is available in the health room, but can also be downloaded The medication must be in the original bottle or packagingAPS does not provide any over the counter medication.  If the over the counter medication is taken for more than five consecutive school days, a physician's authorization must be obtained. 

Asthma Action Plan- Students with Asthma who need medication at school need this form on file in the health room and signed by their doctor.

Allergy Action Plan-Students with food allergies who require medication at school treat a food reaction need to have this form on file in the health room and signed by their doctor.

Students with Diabetes who need several forms on file in the health room. Please come to the health room and speak with the nurse.

Medication Destruction:

****** A parent/guardian will be notified to pick up any medication that is no longer being used by the student          due to discontinuation of a medication or the school year is ending. From the time of notification, the medication must be picked up within one week.  Any medication that is not picked up from the health room will be destroyed according to the APS policy.
