PETROGLYPH HEALTH ROOM

The health room staff would like to welcome everyone to Petroglyph Elementary School. The goal of our health room is to promote optimal health

for each student which will enhance their learning. We ask that every morning our student eat breakfast at home or in the cafeteria through the

APS cafeteria meal plan so that our student stay healthy and can grow physically as well as academically!!!!

HEALTH INFORMATION

PLEASE take some time to complete your student’s health card (front and back). The information and medical history will allow us to be informed of any concerns regarding your child and allow us to provide them with the best of care. We also ask that accurate and most current telephones numbers are listed so that we may contact you if your child is ill or needs further medical care. To ensure they receive optimum care, it is important to update us of any contact changes, changes in your child’s health, current or new medications, or anything else them may impact his or her education.
