1. First, share your concerns with parents and/or guardians, former teachers, and Support Staff. Find out what worked for them when they interacted with this student.
Document all interventions with start and end dates and results on the APS School Support Team Documentation form located in the Workroom. Check out Pre referral Intervention Manual (PRIM) from STAT chairperson (Jenny Star 2001-02). Remember to write your name along the left side of the form. Remember to include the student's current LAS score, Reading Level (WIRP), Math level and Writing Level (Brigance). List the goal(s) you have for this student. Collect work samples of writing and math. If you can see sufficient progress for this student, document and date. Make a copy of the documentation form and place in the student's Cum folder and give a copy to the parent. Keep a copy for your files. Single forms are in the workroom.
2. If the student continues to not meet expectations and
you are not observing or measuring sufficient progress, notify the LEVEL
STAT (K,1,2) or (3,4,5) and they will schedule their next available time.
Have student writing and math samples ready to share. Add to or adjust
interventions and document on form as needed with the assistance of the
LEVEL STAT. Allow three weeks for interventions to take
affect. Document start and end dates and results of interventions.
The LEVEL STAT will make a decision to continue or adjust interventions
or recommend teacher proceeds to the next level of
Support Team.
3. If the LEVEL STAT recommends review by LAVALAND STAT, the chairperson needs the 3 ply APS Support Team Documentation Form and a copy of the data card with current address. Does the parent require the information in Spanish? The chairperson will give the teacher a language appropriate Support Team notification form to fill out. Fill out the form, but do not sign or date. Return to chairperson as soon as possible so the chairperson can date it and schedule for the next available time.
Parents will be notified by mail. Look for the schedule in your mailbox, and posted in the Lounge, Workroom and Office. Be on time to your scheduled appointment and have current information on the student. Bring current writing and math samples, and Reading level (WIRP). Be clear on the goal you expect for the student. The LAVALAND STAT will make a decision and recommendations will be mailed to the parent.
LAVALAND STAT may recommend screenings or observations to determine if the student needs further testing. The chairperson notifies the Evaluation Specialist of the group's recommendations. The ES requests staff members to conduct the screenings and observations. Once the chairperson has received the screenings and reports of observations the LAVALAND STAT meets again to determine if the screening results indicate further testing is recommended. The group can then recommend a full diagnostic evaluation.
4. The Evaluation Specialist will notify the LAVALAND STAT chairperson to supply the teacher
