Use of Automated External Defibrillators
Albuquerque Public Schools is committed to the health and safety of its students, staff, parents, and visitors.
In the event of sudden cardiac arrest-the condition in which the heart stops or fails to produce a life-sustaining rhythm, the combination of CPR and early defibrillation-within the first 3-4 minutes- has been recognized as a significant factor in survival.
The Automated External Defibrillator (AED) is a portable device designed to induce electrical stimulation to the heart muscle in the event of potential cardiac arrest. The AED is capable of recognizing cardiac arrest that will respond to defibrillation, ventricular fibrillation, rapid ventricular tachycardia, and is capable of determining whether defibrillation should be performed. Once determined, the AED automatically charges and delivers an electrical impulse to the victim’s heart. The AED is not used to monitor heart rhythm.
State statute encourages the use of the AED by those trained and provides certain requirements for entities establishing a defibrillation program.
The physician medical director is a licensed physician under contract to Albuquerque Public Schools responsible to provide overall quality assurance and professional medical oversight for the district’s AED program. To fulfill his/her responsibilities, the physician medical director shall:
- Provide a medical prescription for the purchase of an AED;
- Recommend training programs;
- Provide post-event review of each use of an AED;
- Guide and assist the implementation and maintenance of the Albuquerque Public Schools AED program;
- Document for Albuquerque Public Schools that the physician is providing the required medical oversight required by state statute;
- Answer questions or concerns about the AED program; and
- Guarantee confidentiality of all patients.
AED Program Coordinator
Director of Risk Management, or his/her designee, shall be AED program coordinator. The AED program coordinator shall manage both system-wide and site-based components of the AED program. The AED program coordinator’s duties and responsibilities shall include, but not be limited to:
- Ensuring compliance with requirements/regulations of state statute and regulation;
- Communicating with the physician medical director;
- Coordinating the training of district employees to be trained responders and maintain documentation of such training;
- Maintaining a list of district employees who are trained responders;
- Supervising the maintenance of each Albuquerque Public Schools AED device;
- Coordinating and participating in post-reviews of any use of a Albuquerque Public Schools AED and document the use of the AED; and
- Providing an annual written system readiness review of the AED program.
The site administrator shall:
- Ensure the site emergency response plan includes information on how to respond to a medical emergency, including but not limited to the following:
- Internal reporting of a medical emergency to the AED site coordinator, or his/her designee;
- When office staff and/or others should contact a trained responder to respond to a medical emergency;
- When office staff and/or others should alert 911 that there is a medical emergency if such contact has not already occurred;
- Assignment of someone to meet emergency medical service vehicles and to direct EMS personnel to the specific site of the emergency; and
- Completion of appropriate accident report forms.
- Ensure AED is in working order.
- Participate in post-event reviews.
Authorized AED Users
The AED may be used by:
- Employees including administrators, nurses, athletic/activities director, athletic trainers and office staff who have successfully completed an approved CPR/AED training program within the last two years and has a current successful course completion card.
- Students, parents and third-party visitors who are trained responders are also authorized to use an Albuquerque Public Schools AED in an emergency situation.*
The duties and responsibilities of district employees who are trained responders shall be:
- Respond to sudden cardiac arrest events and use an AED in accordance with the guidelines of the approved CPR/AED training program.
- Call 911, and as soon as possible after using the AED, contact the site administrator.
- After use of an AED, submit a written report per this procedural directive.
- Participate in applicable post-event reviews of use of any AED per this procedural directive..
- Follow the provisions of this procedural directive and keep current in certification as a trained responder.
Location of AED Devices
Albuquerque Public Schools shall initially install one (1) AED at each district site in accordance with federal and state statutes. At each site, the site administrator shall determine the location of placement for each AED. The goal shall be to maximize the availability of the AED to all areas of a site and to minimize response time to a cardiac arrest event. Each AED shall be stored in an unlocked alarmed wall cabinet clearly marked as containing an AED.
Any employee trained responder may request permission from the site administrator to temporarily relocate an AED due to a school activity. When doing so, the employee trained responder shall securely place a sign on the AED cabinet that clearly identifies the location of the AED, specifying the date and time the AED was removed, the date and time the AED will be returned, and the name of the trained responder responsible for the unit.
Multiple signs shall be displayed throughout a site that has an AED identifying the site as being equipped with an AED.
Maintenance of the AED
All equipment and accessories necessary for support of medical emergency response shall be maintained in a state of readiness.
The AED program coordinator shall take reasonable measures to ensure that:
- Each AED cabinet is stocked with an emergency unit and the customary supplies necessary to operate an AED including one (1) set of defibrillation electrodes connected to the device. One (1) resuscitation kit shall be connected to the adult pads of each AED. This kit shall contain one (1) set pediatric electrode pads, one (1) set of adult electrode pads, two (2) pairs of latex-free gloves, one (1) razor, one (1) set of trauma shears, and one (1) facemask barrier device.
- Each AED shall be maintained and tested according to the manufacturer’s guidelines as outlined in the operating instructions. Documentation of the manufacturer, the manufacturer’s guidelines and required testing for each AED and written documentation that each AED has been maintained and tested accordingly. - Following the use of an AED, all equipment shall be cleaned and/or decontaminated as required. If contamination includes body fluids, the equipment shall be disinfected.
- Following the use of an AED, the AED shall be delivered to the director of Risk Management or his/her designee to download data and sent to the physician medical director for review. The school shall receive a loaner AED until the AED data is reviewed and the machine is cleaned and disinfected and supplied with all required equipment.
Post-Event Review and Documentation
The trained responder who used the AED shall complete a Medical Event Report by using the Albuquerque Public Schools Student Accident Report Form as soon as possible, but no later than two (2) business days after the event. The trained responder shall give the original of the form to the AED program coordinator and a copy of the form to the site administrator. The Medical Event Report shall contain the following information:
- Date and time of the incident and name of patient;
- Name of person who determined patient’s unresponsiveness;
- Time that 911 was called;
- Number of times the patient was defibrillated;
- Any additional information the physician medical director requests; and
- An Albuquerque Public Schools Police report and/or an Albuquerque Police Department report if requested after the event.
The AED program coordinator shall maintain a copy of all the documents referenced in this procedural directive, including but not limited to documents concerning training, maintenance and use of an AED, for a period of seven (7) years, pursuant to the record retention procedural directive.
The AED performs a self-diagnostic test every twenty-four (24) hours that includes a check of battery strength and an evaluation of the internal components.
Each month, the school nurse or his/her designee shall check the AED for the presence of the green checkmark () and document date and initials on the AED log located in the AED cabinet.
If the green checkmark is not present on the readiness display, the school nurse shall contact the AED program coordinator or his/her designee as soon as possible for maintenance. The school nurse shall notify the AED program coordinator or his/her designee of the need for a loaner AED.
If the expiration date on the electrode equipment is near, the school nurse shall notify the AED program coordinator for replacement equipment.
